Person-centered services designed to promote independence, community inclusion, and meaningful growth.
Serving Individuals with Virginia DD Waivers:
Village Roots Community Care, LLC is a DBHDS-licensed provider of non-center-based day support services, including Community Coaching 1:1 and Community Engagement 1:3. We accept referrals for individuals enrolled in Virginia’s DD Waivers when Community Coaching and/or Community Engagement is an authorized or appropriate service identified through the individual’s ISP (Individual Service Plan).
Community Coaching
We provide one-on-one support that helps adults with developmental disabilities build independence and achieve their personal goals. Services may include developing daily living skills, improving communication, increasing community participation, strengthening social skills, and building confidence in everyday activities. Supports are individualized and based on each person's unique strengths, preferences, and goals.
Community Engagement
Community Engagement services provide opportunities for individuals to connect with others, participate in meaningful activities, and build relationships within their community. Through small group experiences, volunteer opportunities, recreation, and community outings, individuals develop social connections, increase independence, and experience greater inclusion.
Transportation
We provide transportation to support participation in scheduled Community Coaching and Community Engagement activities. This helps individuals safely access community outings, volunteer opportunities, recreation, and other meaningful experiences that promote independence and community inclusion.
Toni Lemons
Founder & Executive Director
Toni Lemons is the Founder and Executive Director of Village Roots Community Care, LLC (VRCC), where her passion for education, advocacy, and community serves as the foundation for helping adults with developmental disabilities live more independent, connected, and meaningful lives.
With a professional background as a Special Education Teacher and a Master’s degree in Education Administration, Toni brings both hands-on experience and leadership expertise to her work. Her years supporting individuals with diverse learning and developmental needs strengthened her belief that every person deserves to be recognized for their abilities, given meaningful choices, and provided opportunities to grow beyond the limitations others may place on them.
Toni founded Village Roots Community Care with a vision of creating services that extend beyond traditional support. She believes meaningful care happens in the community, through relationships, new experiences, skill development, personal choice, and opportunities for individuals to discover what independence means for them.
Her deep love for her community continues to shape VRCC’s person-centered approach. Toni is committed to building an organization where individuals and families feel known, respected, and supported, and where every person is encouraged to develop confidence, build meaningful connections, and become an active participant in the community around them.
